L3 Lessons from Acute Infection and Relevance to Vaccine Development

In my presentation, I will review analyses of sexual mucosal transmission of HIV-1 in an animal model and the early stages of HIV-1 infection in lymphatic tissues (LTs) relevant to prevention or moderation of infection by vaccines and microbicides. These analyses provide evidence for expansion locally at the portal of entry prior to systemic dissemination, the importance of resting as well as activated CD4 T cells in the propagation of infection, and the difficulty virus-specific CTLs likely have in eliminating infected resting T cells. Collectively, these analyses highlight challenges and opportunities for prevention and control: the challenges of the rapid establishment of reservoirs of persistently infected cells and the opportunities for barrier strategies and vaccines that induce a strong mucosal immune response to prevent or contain infection prior to or during the initial local expansion phase.
